Admission

For a programmed operation, we advise you carry out the admission formalities before your hospital entry date.
The admissions office is open everyday from 7.30am to 8pm.
You need the following documents:


  • You photo ID card or your passport or your livret de famille.
  • For non-French nationals working in France, you need your residency permit or your passport as well as the documents requested from French nationals.
  • Your Vitale card and/or its attestation or your last three pay slips. For women on maternity leave, you need your 100% payment attestation.
  • Your insurance papers with the payment charge percentages, or your attestation to cover from the Couverture Maladie Universelle (CMU), Aide Médicale d’Etat (AME) or Atout Santé.
  • If you are a veteran, your treatment book in accordance with article 115 of the veteran's pension code for invalidity and war victims.
  • Your "work accident" form, or professional illness form supplied by your employer if you are a victim of a work accident or illness.
  • For EU members, your E11 or E112 form, duly completed.
  • For non-EU members not working in France, you will be asked for attestation from the organisation of embassy of your country.
Personal effects
For your hospitalisation, you must bring your personal effects (pyjamas, dressing gown) and your toilet bag and a towel.

Valuable objects
We advise that you do not bring: valuable objects, jewellery, check books, credit cards, cash…
You can hand them in to the admission office and they will give you a receipt. Without this formality, the clinic cannot be responsible for any loss or theft of your objects.
